A 72 YO Hispanic male presents with complaints of blurry vision OD x 8 to 12 months. He has a history of ECCE OU, a stroke 4 years ago, and DM type 2 x 10 years. He doesn’t check his blood sugar because he cannot find his glucometer. He has a “long list” of medications, but denies any medical history beyond diabetes.

The patient was referred to retina, where he no-showed. A report was sent to his PCP, and the patient was lost to follow-up.
